I'll report back when I start seeing her. The guy at the gym told me the PT is going to be a lot of strength training with light weights. He said the main thing to do is strengthen the muscles around the injury, which is something I can do in the gym. But I'm still gonna get her to help me out some.

Man, go see someone or talk to someone about it. That could be something more serious. Better to be sure than have something go terribly wrong.

Need to get it checked out but not looking forward to the diagnosis/treatment/rehab.

I'm honestly not trying to be rude or arrogant about this but just offer some advice that worked for me. But that almost sounds like a form issue. I never really feel my shoulders during skull crushers as my triceps are the only real thing burning aside from elbow soreness.
I used to feel my shoulders a lot after triceps day but worked out with a bodybuilder for several weeks and he corrected my form on almost everything. It's been like night and day since then.
